What the contact form collects

The contact form asks for your name, work email, company or operation, role, province or territory, approximate case-count range, and an optional message.

Purpose

Lumeval uses this information to respond to pilot inquiries and decide whether a scoping conversation is appropriate.

Form provider

Contact form submissions are processed by the form provider, Formspree. Please do not submit personal health information, claim details, or identifiable worker information through the form.

No sale or sharing

Lumeval does not sell personal information and does not share contact form information for unrelated marketing or advertising purposes.

Retention

Inquiry information is deleted when it is no longer needed for the inquiry.
